Welcome to the TouchPoint Admissions Department!
To access TouchPoint services, please complete the following steps:
STEP 1: Complete a TouchPoint Application.
Download the TouchPoint Application by clicking here, or contact the Admissions Department to have one mailed to you. After completing the TouchPoint Application, please refer to the bottom of the third page of the Application and provide all available information as is appropriate for the applicant. It is especially important to provide a copy of the applicant’s Social Security Card as well as a copy of a document that verifies the applicant’s diagnosis (this can be a document from the applicant’s diagnosing specialist). An applicant does not need to have a diagnosis of any kind to access TouchPoint services or to benefit from them.
*In order to be considered eligible to receive selected grant funding opportunities available for certain consultation services provided through TouchPoint, this information is mandatory, but is not a guarantee for access to grant funding.
STEP 2: Complete and Sign the "Acknowledgement of Receipt of Notice of Privacy Practices" (page 1) of the HIPAA Policy information.
STEP 3: Mail all completed and signed documents to:

Upon receipt of the application (and attachments), an Admissions Specialist will contact you to verify its receipt and to discuss the next step(s) toward accessing TouchPoint services.
